What is Assist-Life Stage, Transition?
Major life changes like finishing school, moving homes, switching providers, or stepping into adulthood can feel overwhelming without the right support in place. Our transition service helps participants and families plan every change carefully so nothing important gets missed.
We start early, coordinate every moving part, and stay in close contact with everyone involved. Transitions go wrong when planning starts too late, and we make sure that never happens.
